What Are ACL and PCL Injuries?

The ACL (Anterior Cruciate Ligament) and PCL (Posterior Cruciate Ligament) are the main stabilisers of the knee. They prevent excessive forward and backward movement of the tibia and maintain knee stability during sports and daily activities.

Injuries to these ligaments often occur due to twisting, sudden stops, falls, road accidents, or direct blows to the knee.

Causes

  • Sudden change in direction
  • Jumping or landing awkwardly
  • Pivoting in sports like football, basketball, badminton
  • Trauma from accidents
  • Dashboard injury during car accidents

Symptoms

Common symptoms include

  • Sudden knee pain
  • A “pop” sound at the time of injury (common in ACL tears)
  • Rapid swelling
  • Instability or knee “giving way”
  • Difficulty running or climbing stairs
  • Limited movement due to pain

Diagnosis at Our Clinic

Diagnosis is made through

  • Detailed ligament examination
  • X-rays to rule out fractures
  • MRI to confirm ligament tears and associated meniscus/cartilage injuries

Dr Rahul Singh specialises in evaluating complex multi-ligament knee injuries.

Treatment Options

1. Non-Surgical Management

Suitable for partial tears or low-activity individuals.

  • Physiotherapy to strengthen surrounding muscles
  • Bracing for stability
  • Activity modification
2. Surgical Treatment (Arthroscopic Reconstruction)

For complete tears, athletes, or unstable knees, surgery is recommended.

a. ACL Reconstruction

Reconstruction using grafts (hamstring, patellar tendon, or allograft). Results: Excellent stability and return to sports.

b. PCL Reconstruction

Technically demanding but highly effective procedure for restoring stability.

c. Combined Ligament Reconstruction

For cases involving ACL, PCL, MCL, or LCL injuries—often seen after accidents.

Advantages of Arthroscopic Techniques
  • Keyhole incisions
  • Faster recovery
  • Precise ligament placement
  • Minimal tissue damage
